St. Elmo CUSD #202 State of Illinois Health Requirements for the 2024-2025 School Year

Requirements by Grade Level

Please turn into the school office by the start of the school year

Preschool Students (PK)
• Physical Exam (Parents please complete the Health History portion, sign, and date). The exam must be completed no more than 365 days prior to the start of the school year.
• Complete Immunization Record verified by a healthcare provider.
• Lead Questionnaire or Lead Screening

Kindergarten Students (K)
• Physical Exam (Parents please complete the Health History portion, sign, and date). The exam must be completed no more than 365 days prior to the start of the school year.
• Complete Updated Immunization Record verified by a healthcare provider.
• Dental Exam on IL form due May 15, 2025 (dated within 18 months of May 15th deadline)
• Eye Exam on IL form dated within one year of the first day of school

Second Grade Students (2nd)
• Dental Exam on IL form due May 15, 2025 (dated within 18 months of May 15th deadline)

Sixth Grade Students (6th)
• Physical Exam (Sports physical NOT acceptable, IL Certificate of Child Health Exam form required). Parents, please complete the Health History portion, sign and date.
• Complete Updated Immunization Record verified by a healthcare provider, including (1) Tdap, (1) Meningitis - these are 6th grade requirements.
• Dental Exam on IL form due May 15, 2025 (dated within 18 months of May 15th deadline)

Ninth Grade Students (9th)
• Physical Exam (Sports physical NOT acceptable, IL Certificate of Child Health Exam form required). Parents, please complete the Health History portion, sign, and date.
• Complete Immunization Record verified by a healthcare provider.
• Dental Exam on IL form due May 15, 2025 (dated within 18 months of May 15th deadline)

Twelfth Grade Students (12th)
• Complete Immunization Record verified by a healthcare provider, showing proof of receiving (2) Meningitis: First dose received on or after the 11th birthday; 2nd dose on or after the 16th birthday. An interval of at least eight weeks after the first dose. Only one dose is required if the first dose was received at 16 years of age or older.

ADDITIONAL INFORMATION:
Religious/Medical Exemptions: If your child does not receive immunizations for religious reasons, a parent and health care provider must complete the “Illinois Certificate of Religious Exemption to Required Immunizations and/or Examinations” form prior to the first day of school.

Sports Physicals: If your child (Grades 5-12) will be participating in extracurricular sports or activities, they must have a sports physical on file prior to the season’s first practice. 6th and 9th grade students may use the required IL Certificate of Child Health Exam form in lieu of a sports physical, however a sports physical CANNOT be accepted as proof of the IL Certificate of Child Health Exam Form required for those grades. Sports physicals are valid for 395 calendar days from the date the physical was issued.

New Students: New students enrolling in an Illinois school for the first time will be required to obtain a physical examination, vision exam, and provide an up-to-date immunization record regardless of grade level.

Medical Authorization Form: If your child needs to take medication during school, a new medication authorization form needs to be completed each school year. No medications will be given to students without proper authorization from a primary care provider. This includes over-the-counter medications (Tums, Motrin, Tylenol, Midol, cough drops, etc).

Asthma: Any child that has ASTHMA marked on their medical problems list needs to have an inhaler brought to school, Medication Authorization Form on file, and an Asthma Action Plan filled out by your physician and turned in to the office.

Allergies: Any child with ALLERGIES to things such as peanuts, eggs, milk, etc. that have been diagnosed and confirmed by a doctor that require an Epi-Pen/Auvi-Q, must have one at school in case of an emergency, as well as an Allergy Action Plan filled out by your physician and turned in to the office.

Epilepsy/Seizure Disorders: Any child with EPILEPSY/SEIZURE DISORDERS needs to have a Seizure Action Plan filled out by your physician and turned in to the office.

Food Substitution Form: If your child requires a food substitution due to a medical condition or food allergy, a new food substitution form needs to be completed each school year and signed by the child’s primary care provider.
